Ontario Phillips Obituary

Ontario Lorenzo Phillips Sr., better known as Terry, was born Sunday September 16, 1951 in Chicago IL to Delores and Homer Phillips. His parents both preceded him in death.

Ontario served in the military and later worked for the Chicago Transit Authority.

On Saturday September 12, 2020, Ontario made his transition. He is survived by two daughters, Perpetrial, aka Pat/Necee, and LaToia; two sons, David Sr. and Ontario Jr.; grandkids Bria, Braelyn, David Jr., Zaria, William, Chase and Caleb; his siblings, Anthony, Tyrone and Regina; his nieces, Carmen and Danielle; his nephew, Tony; and a host of great nieces, great nephews, cousins and friends.
